You can fill these forms out on your computer, then print or scan a copy for mailing (you cannot save data typed into the form unless you have a PDF authoring application such as Adobe Acrobat).  If you have trouble typing into the form, make sure your PDF reader is set to view documents in PDF mode, rather than PDF/A mode.  In Adobe Reader, go to Edit>Preferences>Documents and set the PDF/A View Mode to “Never.”

ACAC is committed to protecting the privacy of each of its customers. When you pay your invoice on our website, we collect the following information:

Your credit card information. This data is transmitted from your computer to our servers and back again using advanced SSL (secure socket layer) technology. It is temporarily stored in a secure database until your order is processed, at which point it is deleted permanently.

Your name, phone number, billing/shipping address and e-mail address. This information is required for the completion of your order. We do not distribute your contact information to any individual, company or organization.
